Types of Disability Placards

Disability Placards are issued to disabled individuals and can be used in any vehicle. Depending on the severity of your disability, you can apply for one of the two types of placards listed below. Your physician can help you determine which is more appropriate:

Temporary

If your doctor selects the temporary placard option, they’ll also include the expiration date (a maximum of 6 months from the date your application is processed).

5 Year

Valid for 5 years from the processing date.

Note: We no longer offer lifetime permanent disability placards.

If you wish to obtain a disability license plate, you must first have a 5-year disability placard. A disability license plate must be renewed every year. To apply for a disability license plate, click here.

How to Apply

Complete your portion online

Select between the options to renew, replace, or apply for a new disability placard. You will also be asked for the number of placards being requested.

Enter your physician's information

Be sure to include your physician’s email. If you do not have this information, please contact your physician’s office.

Submit your application

Once your application is submitted, there is nothing left to do on your end. Your physician will receive an email to complete his/her portion of the form and submit it to Service Oklahoma.

Note

Disability placard applications expire 60 days past the date of the physician's signature.

Receive your placard by mail

Once your physician submits their portion of the application, you should expect to receive the placard to the address provided in your application. Due to high application volumes, please allow up to 60 days to receive your placard.

At Your Physician’s Office

Submit a digital application at your next physician visit. At your next appointment, click the "Apply with Physician" button to submit a digital application. This can only be completed with your physician present.

What is the turnaround time for a Disability Placard?

You can expect your disability placard within 20 business days of submitting your application.

To ensure proper delivery, please make sure that you’ve given us your correct mailing address. Also, be sure that you’ve included any apartment numbers, suite numbers, or box numbers in your return mailing address. If you haven’t received your disability parking placard after 20 business days, you’re welcome to call the Disability Parking Permit Desk at (405) 522-7000 to check on the status of your application.
